Central Illinois Debt Management & Credit Education, Inc. (CIDMCE) is required to disclose the following information to all debtor students before the debtor student pays for a course:

• CIDMCE has been approved to issue certificates evidencing completion of counseling and/or a personal financial management instructional course in compliance with the Bankruptcy Code. Approval does not endorse or assure the quality of a Provider's service.
• An individual must receive a credit counseling briefing during the 180-day period preceding the date of filing the petition by such individual. To schedule an appointment for face to face Bankruptcy Pre-Filing Counseling, please call 1-888-671-2227 for an appointment.
• The online bankruptcy course is available to debtor students 24/7 via Internet access or you may choose to participate in a classroom course. If you are interested in a classroom course, please call 1-888-671-2227 to received class dates, times and locations.
• The debtor student is asked to contribute a course fee of $50 per individual or $75 per couple, whether the instruction is provided online or in a classroom environment. Please call 1-888-671-2227 for classroom registration options.
• Scholarships are offered in the event the cost of the class is a hardship to you and your family. Please contact us at 1-888-671-2227 for further details.
• The bankruptcy course is provided to debtor students without regard to the student's ability to pay the stated fee.
• CIDMCE does not pay or receive fees or other consideration for the referral of debtor students.
• Our counselors have been certified through the Association for Financial Counseling and Planning Education (AFCPE) and have a minimum of one year work experience in personal finances, budgeting and debt management.
• At the conclusion of your credit counseling session, you will be provided with a Certificate of Counseling that you will need if you are filing bankruptcy. Upon completion of the debtor education course you will be provided with a Certificate of Debtor Education.
• We will not disclose any such information that you provide orally or in writing to anyone, except as authorized by you in writing or required by law. We may compile data in aggregate form; however, your information will not be disclosed in any manner that would personally identify you.
• If you are dissatisfied with the service provided by our agency, you can utilize the Complaint Resolution Process. Nothing herein shall apply to actions or claims under the provisions of the United Stated Bankruptcy cod, 11 U.S.C. Sec. 101 et seq.
• I certify that all information I provide is true, complete, correct and made in good faith. I also certify that I personally will complete the education program. I understand that knowingly making a false or fraudulent statement or misrepresentation about my identity or completion of the bankrupty education course is a violation of the requirements of Federal Law.
